I think documents with no tests or with tests that were never executed should be in white (or grey), but not green. Green has a positive meaning. Documents with no tests should not be associated with that meaning. So, I would stick with 3 colors:
- red : last execution did not pass
- green : last execution passed
- white/grey : no tests OR never executed

as for suggestions;
- a little 0 (zero) as superscript to the red pepper, corresponding to the fact that the test has passed 0 times.
- a "warning" symbol in a lower part of the red pepper meaning that this test has something special

more suggestions will be added as they come to mind!

I'm not sure that adding a new color for a new status is the way to go. You don't want to end up with a rainbow colored pepper because you ran out of colors.

In my mind, there is no real difference between a test that never passed from a test that previously passed. Both should be red. To show the difference between the two, I would display the same red pepper icon, but slightly modified to identify the different status. This way, as a user, I see red or green peppers. Their meaning stays clear.
